The tear troughs are the hollows that run beneath the eyes. As they deepen with age or genetics, they cast a shadow that reads as dark circles and a tired, drawn look, no matter how well rested you are. Tear trough filler softens this by restoring gentle support to the hollow, smoothing the step between the lower eyelid and the cheek.

We use Teosyal Redensity II, a hyaluronic acid filler formulated specifically for the delicate under-eye area. Because this is a high-precision region close to the eye, it is a treatment that genuinely benefits from being carried out by an experienced medical injector, with careful assessment of whether it is right for you. At Āatma Aesthetics London, every treatment is administered by a GMC-registered doctor inside our CQC-registered clinic at The O2, Greenwich.
The Approach

A High-Precision Area, in Expert Hands

The under-eye is one of the most delicate and demanding areas to treat, which is why assessment and technique matter so much. Dr Shaan first confirms that filler is the right choice for your under-eyes, then places small, carefully measured amounts of Teosyal Redensity II deep against the bone to support the hollow, rather than overfilling it.

This conservative, medical approach is what produces a refreshed, natural result and keeps the treatment safe. Hyaluronic acid filler is also reversible if ever needed, which is an important reassurance in such a sensitive area.

How tear trough filler works

Smooth the Hollow, Brighten the Eyes

With age and genetics, the face loses volume and support under the eye, and the tear trough hollow deepens. Light falling into that hollow creates the shadow many people mistake for dark circles, along with a tired, sunken look. Placing a small amount of hyaluronic acid filler deep within the hollow restores gentle support and smooths the transition into the cheek, so the shadow softens.

Teosyal Redensity II is chosen specifically for this area because it is formulated to sit well under the thin under-eye skin and is less likely to hold water and cause puffiness than fillers designed for other parts of the face. Results are visible immediately and settle over the following two weeks, and because the filler is hyaluronic acid, it can be dissolved if necessary, an important safety feature here.

Treatment is carried out at our doctor-led clinic inside The O2 in London.

Is Tear Trough Filler Right for You?

Tear trough filler is ideal for:

-Hollows or shadows under the eyes that look like dark circles

-A tired, drawn appearance even when well rested

-A visible step between the lower eyelid and the cheek

-Mild to moderate under-eye volume loss

-Wanting a refreshed, brighter-eyed look

Tear trough filler is not suitable for everyone. Pronounced eye bags, fluid retention, or very thin or pigmented under-eye skin can make filler unsuitable or risk puffiness, and dark circles caused by pigment rather than shadow will not respond to it. Dr Shaan will assess you honestly at consultation and will tell you if you are not a good candidate or if another approach would serve you better.

Meet Dr Shaan Patel

Dr Shaan founded Āatma after years of medical training at the University of Southampton and clinical experience across hospitals in the UK and Australia. That foundation shapes how he practises aesthetic medicine: methodical, anatomically led, and conservative.

His approach is built on a simple rule - treat what's in front of you, not what's in fashion. That means honest consultations, treatment plans that prioritise long-term face shape over short-term fixes, and a willingness to say "no, not yet" or "no, not this" when it's the right answer.

"My goal is to keep you looking like you - naturally refreshed, never overdone."

FAQ

Got questions?
We’ve got answers

Will it get rid of my dark circles?

It depends on what is causing them. If your dark circles are shadows cast by a hollow, filler can soften them considerably. If they are caused by pigment in the skin rather than shadow, filler will not change them, and a different approach is needed. Dr Shaan will identify which type you have at consultation and advise honestly.

Does it hurt, and is there downtime?

Discomfort is usually mild. Some swelling or small bruises can occur for a few days afterwards, as this is a delicate area, but most people return to normal activities straight away and any bruising is easily covered.

How long does it last?

Results in the tear trough typically last around nine to twelve months, often longer than filler in more mobile areas, because there is little movement here. A top-up maintains the result when it begins to fade.

Is it safe, and what if I do not like it?

In experienced medical hands and with careful assessment, tear trough filler is a safe treatment, and being carried out by a GMC-registered doctor minimises the risks of such a delicate area. Because the filler is hyaluronic acid, it can also be dissolved with an enzyme if you are unhappy or if any correction is needed.

Am I a suitable candidate?

Not everyone is. The best results come in people with genuine hollowing and good-quality under-eye skin. Pronounced eye bags, fluid retention, or very thin or pigmented skin can make filler unsuitable. This is exactly why assessment comes first, and Dr Shaan will be straight with you about whether it is right for you.

How is it different from Sunekos?

They address different things. Tear trough filler restores volume to fill a hollow and soften shadows. Sunekos improves the quality of the under-eye skin itself, helping with crepiness and fine lines, but does not fill a hollow. Some people benefit from one, some from a combination, and Dr Shaan will advise what suits your under-eyes.
